Not in my personal life, but I do this a lot in my professional life. It's something I seem to do naturally with patients, especially those who seem a bit confused. However, although I said "in my professional life", I actually don't use the names of my colleagues or seniors unless there's no other way to get their attention. I'm much more likely to sidle up and try to make eye contact.

Usually only at work. I think mostly, it's creepy.

I'm required at work to say the customer's name at least once. I think it's worth something like two points on an audit. Some of my co-workers have mocked this rule at times, overusing the name to the point of ridiculous-ness.
