Wait, you ask the question "Natural or do everything you can to look nice", and you have the answer "Natural" but not the answer "do everything I can to look nice"? You don't even have both the answers that are no-matter-what needed!

SC, but it isn't about make-up. Even guys who don't wear make-up can put a lot into their looks, for example using steroids to pump up, rather than "naturally" building their muscles.

I'll have to say both because some guys don't wear make-up,so it wouldn't really apply to them, however they can say whether they would want their wives or girlfriends to wear make-uo etc. For woman, it can be for either.

I changed it to nice, I know some men who wear make-up as well. I don't know any that have the acrylic nails etc. However, men can say whether they would prefer their girlfriend or wife to be "natural" or not. I didn't think this was going to confuse anyone, I can see that this is a very difficult question to get across. I'll see what responses I get after this, I might just delete the survey. Thanks!

Men wear makeup as well sometimes (I'm not referring to actors). I've known men with scars who use makeup to conceal them. Maybe if you got rid of the word "beautiful" which has more of a feminine connotation and find a more gender non-specific word. Unfortunately, I can't think of one at the moment. Maybe this will generate some suggestions.

I'm trying to make it as clear as possible, if this isn't clear please leave me a sample sentence. Some people don't like make-up, acrylic nails, and hair extensions etc. Some prefer natural beauty, I'm asking who prefers what.

Good, although I think there does need to be a semi-colon after "a little help" - that sentence is a little confusig the way it is.
Also, perhaps another option like "It's okay do to anything to improve your appearance"
