"When is it too late for an abortion?" is more the title than the question. I prefer that survey titles always reveal their contents, in essence at least.

"Which of these is an acceptable time frame for an abortion?" is not so great a title, but the first question phrasing doesn't work with the options.

if you are going to rephrase in the explanation then just rephrase the question completely. you are asking opposing questions this way and it is confusing!!

I don't know if your choice of language is from being in the medical profession or not living in the US. Seems it would complicate phrasing the question which I expect to be easily understood.

There is such a term 'Fourth trimester abortion'?! I hope it means something different than I meant.

You might want to say what you mean by "4th trimester" as I've just had to look it up. You might also want to specify that you mean "abortion" in a colloquial sense (i.e. a "termination") rather than a medical one.
