I'm not sure if your intention is to see who wants to do what as opposed to who wants to watch what by asking them for two answers, one that would be interested in doing and the other that you would be interested in watching. With the results you wouldn't be able to tell who picked what to watch and who picked what to do.

Like I said I don't know if that part is important to your survey.
