If you are trying to make a survey about human induced global warming you need to be more specific. Global warming is a fact. It reversed ice ages for millions of years and before humans arrived. What is in dispute is whether or not humans influence global temperatures. If this is what your survey is about you shoud add "human induced" or "anthropogenic" before "global warming".

It is precisely because of surveys like this that the other option is needed. If you clarified the question as I suggested you may not need the 'other' option but right now it is necessary.

This should be multiple pick because we might think or not think it exists, but not know what "horse hockey" is. You should also have a "No, I don't think it exists" option as well. I realize you do have that second option, but you're also assuming that everyone is familiar with the term "horse hockey" as well.
