Hmm... well, it's not 'opinion', I mean, it's a testable hypothesis if you supply an operational definition for 'mellow', which I think could simply be taken to mean a general shift from type a to type b personality. Research I've seen on the subject isn't, ugh, definitive, but seems to suggest that (contrary to what you assert) that there isn't any real correlation with that type and age.

I think having multiple questions is okay as long as there are options to cover them. You don't have them - you have the options in answer to the second question, but only one option in answer to the first. This could be really interesting with a bit of clearing up.

please only one question as the question. the rest can go in the explanation box. options should relfect answers to the question. either make it 'people' or 'I' know what i mean?
also need option for 'other' and/or 'i dont know'.
